The United States Department of Labor’s Occupational Safety and Health Administration (OSHA) is currently investigating a workplace accident that cost the life of one man on Thursday, June 23 when he was pulled through an industrial machine in southwest suburban Plainfield, Illinois.

Police officers responded to the scene of the accident shortly after 1:30 p.m. at Coilplus Inc. in Plainfield. Police said 50-year-old Joliet resident Carlos Rivera had been trapped in a steel baler.

The Will County Coroner’s Office declared Rivera dead at the scene of the accident at around 2:15 p.m. An autopsy on Thursday showed that Rivera died of multiple injuries to his body.
